JMJ Fertility Specialists & Surgeons is an out-of-network practice. This does not mean your coverage cannot be used; we provide the documentation needed to help you maximize your out-of-network insurance benefits. This allows us to provide comprehensive, individualized treatment based on your needs—not the limitations of an insurance plan.

 

Patients pay JMJ directly for care. We will explain our fees before treatment and provide the documentation you need to seek reimbursement through any out-of-network benefits available to you.

Why Is JMJ Out of Network?

Infertility, endometriosis, recurrent miscarriage, pelvic pain, and other complex gynecologic conditions rarely have a single, universal solution. A cookie-cutter approach will often fail to treat the root cause of the disease.

 

Remaining out-of-network allows us to devote the time and expertise needed for detailed evaluation, restorative fertility treatment, and meticulous fertility-conscious surgery. Our personalized, tailored approach ensures you receive the highest level of care. Our state-of-the-art in-house imaging and hormone lab provide same-day answers.

Using Your Insurance Benefits

Many patients receive reimbursement through their out-of-network benefits. The amount depends on your individual plan, deductible, coinsurance, and the insurer’s allowed amount.

 

Our team will gladly provide an itemized receipt, or superbill, with the information needed to submit a claim. We also encourage you to contact your insurer before care so you understand the benefits available to you.

 

Patients without out-of-network benefits are always welcome to receive care as self-pay patients.

Planning for Your Costs

Office services are generally paid at the time of care. Surgical patients receive a written estimate and payment schedule before surgery.

 

JMJ’s surgical estimate generally covers the surgeon’s professional fee. The hospital or surgery center, anesthesia group, pathology service, and any other participating specialists may bill separately. These services may be covered by your in-network benefits, depending on the facility and your insurance plan.
